Die inskrywingsvorm dien as 'n basiese ooreenkoms tussen "Op Die Planke" Fees en die persoon(e) of instansie wat inskryf. Die inskrywer sal streng volgens die reëls van die kompetisie optree en dit nakom. Die persoon(e) of instansies wat inskryf, vrywaar "Op Die Planke" Fees van enige verlies, en of skade van enige aard ookal tydens die hele verloop van die kompetisie. Die persoon(e) of die instansie verleen hiermee magtiging aan "Op Die Planke" Fees om foto's of video's van optredes/produksies van groepe of individuele items te neem en dat "Op Die Planke" daarvan gebruik kan maak vir media, advertensie en om bemarkings veldtogte te loots asook te koop aangebied mag word."Op Die Planke" Fees bied eger die geleentheid aan alle deelnemers om te streef tot nuwe en groter hoogtes.

Entry form serves as a basic agreement between "Op Die Planke" Festival and the person(s) of the institution, which enters. An individual or institution who enters will strictly comply with the rules and regulations of the competition. The person(s) or institution, who enters, indemnify "Op Die Planke" Festival of any losses, damages of any form during the duration of the competition. The person(s) or institution hereby grants permission to "Op Die Planke" Festival to take photos and video footage of all performances and productions of groups and individual item, to be used for media purposes, adverts and promotion campaigns and sales. "Op Die Planke" Festival presents the opportunity to any participant to strive to new heights.
